Q-Zone Waiver Form

1.      I wish to play laser tag at Q-Zone.

2.      I acknowledge that participation in the game involves physical activity that could result in injury to me.  I assume full responsibility for any injuries or damages which may occur to or be caused by me in, or about the game’s premises from whatever cause, including, without limitation, the affirmative negligence of Q-Zone, it’s owners, licensees, employees or agents, and I completely release and discharge Q-Zone, it’s owners, licensees, employees or agents from all claims, damages or other liabilities, present or future, whether or not known or anticipated that may result from or arise out of the use of the laser tag game or the premises, facilities and equipment related thereto.  I have read and understand the foregoing waiver.

3.      I agree to play laser tag according to the rules and instructions given to me by the staff.  I acknowledge that Q-Zone accepts no responsibility for any act or thing done by me during the course of the game.

4.      I accept full responsibility for any damages to Q-Zone premises, facilities, and/or equipment caused by me.

5.      I agree to inform a member of the Q-Zone staff of any medical condition or treatment that I have prior to participating in laser tag.

6.      I agree that all acknowledgments, agreements, and acceptances contained herein shall be in full force and effect each and every time I play laser tag at this facility.
